- Manpower request with salary scale from Japanese Promotions Company addressed to PRA
- Job Order with salary scale from Performance Venue addressed to Japanese Promotion Company
- Model Employment Contract complying with the Minimum Standard Employment provisions of both the Philippines and Japan with the original signature of the Company President/Authorized Representative
- List of Employees
All pages must be signed by the Employer.
- Company Registration with English translation
- Copy of PRA POEA License/Certificate of Renewal
- Passport and/or ID Copy of Company President/Authorized Representative
- Passport Copy of PRA President/Authorized Representative
- Notarized Joint Affidavit of Undertaking and Assumption of Responsibility
- Copy of Escrow Deposit/Agreement
For each performance venue:
- Notarized Service Agreement between the Performance Venue and Japanese Promotion for each
- Notarized Joint Affidavit of Undertaking and Assumption of Responsibility
- Master Employment Contract complying with the Minimum Standard Employment provisions of both the Philippines and Japan with original signature of the Company President/Authorized Representative, Venue Owner, and PRA President.
