IMPORTANT REMINDERS
  1. SpOs that are authorized to act as EA by the MHLW are not required to submit the “Joint Affidavit of Undertaking” and “Job Order.
  2. Pursuant to Section 3, Rule VII of the Omnibus Rules and Regulations Implementing Republic Act 8042 as amended by RA 10022, SpO/EA and PRA are jointly responsible for the welfare of recruited HSWs
  3. All Japanese documents must have English translation.
  4. English translations must be consistent in all documents.
  5. The Master Employment Contract, the Addendum to the Employment Contract and the Joint Affidavit of Undertaking shall use forms downloadable.
  6. Documents that require signature of the Authorized Representative of EA and SpO shall be signed and stamped with the official seal (hanko).
  7. All the pages of the Master Employment Contract and the Addendum to the Employment Contract shall be signed and stamped with the official seal (hanko) by the President/ Authorized Representative/s of the SpO.

Documentary Requirements

A. For Accreditation of Employment Agencies (EAs) and Specified Organizations (SpOs)

 

  1. Special Power of Attorney (SPA) issued by a duly authorized representative of the EA to the Philippine Recruitment Agency (PRA). (notarized)
  2. Recruitment Agreement issued by a duly authorized representative of the EA to the Philippine Recruitment Agency (PRA). (notarized)
  3. Joint Affidavit of Undertaking between the EA and SpO. (notarized)
  4. Company Registration (Tokibo Tohon) of the EA and SpO.
  5. Business Permit issued to the EA by the Ministry of Health, Labor and Welfare (MHLW) for the recruitment and placement business stating the Filipino HSWs are covered in the program.
  6. Business Permit (Tokuteikikan Kijun Tekigou Tsuchisho) issued to the SpO by the Third Party Management Council in the concerned NSSZ approved by the Prime Minister stating that the SpO is authorized to recruit Filipino HSWs.
  7. The document submitted to the Third Party Management Council by the SpO as proof that the Filipino HSWs shall be paid same as the Japanese counterparts in that company.
  8. Manpower request from the EA to PRA indicating the number of vacancies and salary rate/month detailing Gross Pay, Deductions and Net Pay signed and stamped with the official seal by the duly authorized representative of the EA.
  9. Job order from the SpO to the EA (in case SpO is not an authorized EA) duly signed and stamped with the official seal by the authorized representative of the SpO.
  10. Master Employment Contract with Addendum to the Employment Contract complying with the minimum Standard Employment provisions of both Philippines and Japan signed and stamped with the official seal of the SpO authorized Representative.
  11. Interview with the Labor Attaché of the authorized representatives of the EA and the SpO when warranted.
B. For Verification Additional SpOs (New) by accredited /registered EAs at the MWO/DMW

 

  1. Joint Affidavit of Undertaking between the EA and SpO. ( notarized)
  2. Company Registration (Tokibo Tohon) of the SpO.
  3. Business Permit (Tokuteikikan Kijun Tekigou Tsuchisho) issued to the SpO by the Third Party Management Council in the concerned NSSZ approved by the Prime Minister stating that the SpO is authorized to recruit Filipino HSWs.
  4. The document submitted to the Third Party Management Council by the SpO as proof that the Filipino HSWs shall be paid same as the Japanese counterparts in that company.
  5. Manpower request from the EA to PRA indicating the number of vacancies and salary rate/month detailing Gross Pay, Deductions and Net Pay signed and stamped with the official seal by the duly authorized representative of the EA.
  6. Job order from the SpO to the EA duly signed and stamped with the official seal by the authorized representative of the SpO.
  7. Master Employment Contract with Addendum to the Employment Contract complying with the minimum Standard Employment provisions of both Philippines and Japan signed and stamped with the official seal by the authorized Representative of the SpO.
C. For Accredited /Registered SpOs authorized as EAs operating in multiple strategic zones

 

  1. Business Permit issued to the EA by the Ministry of Health, Labor and Welfare (MHLW) for the recruitment and placement business stating the Filipino HSWs are covered in the program in the specified strategic zone.
  2. Business Permit (Tokuteikikan Kijun Tekigou Tsuchisho) issued to the SpO by the Third Party Management Council in the concerned NSSZ approved by the Prime Minister stating that the SpO is authorized to recruit Filipino HSWs.
  3. The document submitted to the Third Party Management Council by the SpO as proof that the Filipino HSWs shall be paid same as the Japanese
    counterparts in that company.
  4. Manpower request from the EA to PRA indicating the number of vacancies
    and salary rate/month detailing Gross Pay, Deductions and Net Pay signed and stamped with the official seal by the duly authorized representative of the EA.
  5. Master Employment Contract with Addendum to the Employment Contract complying with the minimum Standard Employment provisions of both Philippines and Japan signed and stamped with the official seal by the authorized Representative of the SpO.
D. For Verification Individual Employment Contracts for Household Service Workers (HSWs) for the National Strategic Special Zones (NSSZs)

 

  1. Original Certificate of Eligibility (COE) from the Japanese Immigration
  2. Passport Copy of the Household Worker
  3. *Notarized Affidavit of Undertaking of partner PRA stating that the worker to be deployed has the appropriate skill/training and competence to perform her obligations.
  4. *Certificate of Work-Readiness and Orientation issued by the partner PRA
  5. Complete set of Individual Employment Contract with original signatures
  6. Copy of approved Master Employment Contract
  7. Copy of the Accreditation
  8. Accomplished Application Form “MWO-Tokyo Form NSSZ-2019 v1”
